PAT-ORV: operational source-authorization review reliance use integrity review
An append-only protected record of one bounded local review event over one exact PAT-ORI inspection and PAT-ORU receipt. It preserves event type, currentness, timing, exact hashes, commitments, predecessor continuity, and any fresh-use requirement without storing raw reasons, contests, correction text, process input or output, commands, URLs, credentials, private identities, or topology. Review is not repair. Contest is not invalidation. Integrity is not certification.
