An append-only protected record over one exact PAT-ORS suitability decision and its exact PAT-ORQ qualification policy. It preserves exact target hashes, purpose and scope commitments, timing, currentness, review outcome, predecessor continuity, commitment-only findings, and an optional fresh-qualification requirement. It never mutates the policy or decision. Review is not repair. Contest is not invalidation. Currentness is not truth. Qualification is not certification. Suitability is not execution.
